HOUSEPARENT/CASE MANAGEMENT JOB DESCRIPTION

Position: GHF Home Houseparent/Case Manager

Reports to: Executive Director

Employee Status: Full Time

Salary: $16,800-$21,400

BASIC REQUIREMENTS AND QUALIFICATIONS:

  • Demonstrate successful promotion, of and adherence to, the GraceSon Housing Mission Statement, Core Values, and Statement of Purpose, Expectations and Faith.
  • Ability to work with at risk and behavioral challenging youth
  • Maintain a current driver’s license and automobile insurance, which includes medical and liability coverage.
  • Acquire and maintain current certification in CPR, First Aid, bloodborne pathogens, and a food handler’s card.

GENERAL HOUSEPARENT(S) RESPONSIBILITIES – The Houseparent(s) will:

  • Provide a nurturing, grace-filled family environment
  • Maintain an orderly home modeling responsibility, accountability, and healthy living in a grace-based environment
  • Maintain unity throughout the staff, residents, and volunteers
  • Demonstrate respect for all residents, staff, volunteers and guests
  • Maintain confidentiality
  • Operate as part of a program team with the Executive Director and a variety of trained volunteers
  • Houseparent(s) are to be available to the residents during their entire shift
  • Recognize and embrace daily teachable moments as an opportunity to demonstrate and explain God’s love and grace
  • Establish a daily point of connection with each resident as you develop a close relationship with them and their children
  • Provide supervision, direction, and guidance to each resident according to her specific needs and goals

CASE MANAGEMENT RESPONSIBILITIES – The Houseparent(s) will:

  • Work with social service agencies and develop contacts in the community
  • Organize orientations for potential residents
  • Ensure that all documentation is created and maintained for residents
  • Develop and implement a written case plan for each resident
  • Assess progress toward goals
  • Facilitate referrals to community resources including education, medical, dental, and mental health for resident and their children
  • Transport residents in GraceSon vehicle as determined by team
